Contest entry form and rules
1. Review the rules (PDF) This media contest is designed to increase teen on-the-job safety awareness through creative videos or graphic designs that would catch the attention of high school teens and convince them to take the O[yes] online Safety Awareness Training as a great way to learn how to stay safe and healthy on the job. By entering this contest, entrants agree to abide by all rules.
2. Entrants are encouraged to submit entries online. Submissions can also be sent by mail on a USB thumb drive or delivered in person. See the applicable set of rules pertaining to your method of submission in the rules. If you are submitting online and would like to be mailed a hard copy entry form and permission form to fill out, please contact Heather Studnick at heastu@saif.com. Please note that you will need to request forms in advance of the deadline to ensure your submission and forms are all received before 5:00 p.m. on Friday, February 21, 2025.
3. Complete the Permission form if participant is under 18 years of age.
4. Before you submit your entry, you can use this checklist (PDF) to make sure you have met all of the requirements.
